I'm the lead tech at a performance shop. We have anywhere from 5-25 cars a week on the dyno. PLX widebands are almost always dead on with the dyno's Motec wideband. Every single other brand of wideband is almost always off by 0.3-1.3

Check the ecu grounds in the engine harness, and any other grounds you can find.
Make sure you are using Resister spark plugs (R in the plug part number, like BKR7E).
If you have a jumper harness, it may be of bad quality and have a bad wire somewhere.

I would say it's a 90% chance the ecu itself is bad and needs to be repaired or replaced.

if the live data is showing the duty cycle still running 98%, then either the solenoid is faulty or the wiring is faulty

I misunderstood your original explanation. I did think that you were using the ecu to trigger a relay on the pwm circuit, and the relay trigger the solenoid. mac valves work off duty cycle at specific frequencies. Relays can not react fast enough to provide accurate duty cycles or frequencies. The Mac valve needs to see raw the square wave directly from the ecu signal. The only reason to use relays on anything is when the ecu trigger is a low amp circuit...
